Today, six out of ten adults in the US have a chronic disease and four out of ten adults have two or more.  A growing number of doctors are connecting their patients with health coaches as part of their treatment plan for conditions such as diabetes, high blood pressure, and high cholesterol. One key factor that heavily influences chronic conditions is unhealthy lifestyle habits.
